Please carefully read the form below and submit it to confirm your upcoming lash appointment.

I give the certified eyelash extension technician, Christina Towfigh, consent to place and remove eyelash extensions and perform other services such as lash tinting or curling.

I understand and consent to having my eyes closed and covered for the duration of the 30-120 minute procedure.
I understand there are risks associated with having eyelash extensions applied to and/or removed from my natural eyelashes.
I further understand that as part of the procedure: eye irritation, stinging, discomfort and skin irritation can occur. Should I require a physician’s follow-up care, it will be at my own expense.
Should I require the lash extensions to be removed immediately I understand that the original lash service is non-refundable.
I understand and agree to the care instructions provided by the certified eyelash extension technician for the use and care of my eyelash extensions and/or eyelash tint and curl.
I acknowledge and accept the consequences of failure to adhere to these instructions. If I fail to follow the after care instructions I understand that it may cause the eyelash extensions to fall out prematurely and/or damage the extensions and/or my natural lashes.

I am informing the certified eyelash extension technician of the following conditions by checking all boxes that apply:

Current use of contact lenses which I agree to leave in during my eyelash extension application and/or lash tint Current use of eye drops of any kind (prescription or over-the-counter) Current allergies or sensitivities to: latex / fumes / tapes / cleaners / collagen / adhesives / removers History of claustrophobia History of recurrent eye or tear duct infections or styes History of dry eyes Recent history of Chemotherapy (less than 6 months) Other medical conditions which may prohibit or compromise placement and retention of eyelash extensions

I agree to the following eyelash extension maintenance instructions:

No waterproof mascara
No tubular mascara or mascaras that contain fibres to enhance length and thickness
No oil based products around the eye area
No contact with water around the eyes for the first 2 hours after lash application/tint
No continuous pulling or rubbing of the synthetic lashes
Rinse entire eye (including extensions) well after swimming or extreme activity (e.g. hot yoga, spin)
Clean eyelid area with cleanser and/or fresh water before bed to remove excess natural oils that build up throughout the day

Your appointment time is reserved just for you. A late cancellation or missed appointment leaves a hole in my day that could have been filled by another client. With limited spots available due to COVID-19 protocols for sterilizing and prepping my studio between appointments – there are less available spots I can offer my lash ladies. As such, I require 24 hours notice for any cancellations. Clients who provide less than 24 hours notice will be charged the full fee of the missed appointment. If your COVID-19 situation suddenly changes and you need to stay home, you will not be charged. This must be communicated to me prior to your appointment. Please note that no shows without prior communication will be charged.

I am aware of the Cancellation Policy and agree to pay the full fee if I miss my appointment without giving 24 hours notice.(required)

This agreement will remain in affect for this appointment and all future appointments performed by Christina Towfigh at Ooh la la Lashes. I have read and understood that this consent agreement is legal and binding. I have read and fully understand all information in this agreement. I release Christina Towfigh and Ooh la la Lashes from all liability associated with this procedure, which is performed with the utmost attention to safety and proper application using tools and products that the technician has been professionally trained to use. There is no guarantee for the bonding time of the eyelash extensions. I understand that there are many factors that may affect the life of the eyelash extensions such as water and moisture contact, humidity levels, and activities involving exposure to high temperatures and perspiration.

I grant permission to have before and after photos of my eyes taken which may be used for marketing purposes for Ooh la la Lashes.

Accordingly I acknowledge and assume the risk of becoming infected with COVID-19 through this elective service and I give my express permission for the certified eyelash extension technician to perform this service.(required)
